Insurance Questions:

Please contact us at the above number with any questions pertaining to the surgical services provided at our facility.  All other questions should be directed to the appropriate entities (pathology, physician, anesthesia - see below}.

A deductible is a set dollar amount that you pay before your insurance begins to pay its portion of the charges.  A coinsurance is a percentage of the charges that you pay while the insurance company pays the remaining percentage.  When you owe a deductible or coinsurance amount, we will bill you after your insurance company has processed your claim and paid their portion of the charges.  We expect payment in full within 30 days after you receive your statement.

PRECERTIFICATION:

Depending on your insurance plan and carrier requirements, pre-certification, preauthorization, or notification for your surgery/procedure may be required.  This is usually administered by your doctor's office and may not be available to the Center until just prior to your surgery date.  However, in some instances, it may be up to the patient to obtain this information.  Please check with your doctor’s office if you have been preapproved.  It is always wise for patient’s to contact their insurance to verify if the procedure they are scheduled for is covered under their plan.   

For patients with an HMO, IPA, or POS that require a prior approval or authorization (referral), it is ultimately the patient’s or guardian’s responsibility to obtain the referral from your primary care physician (PCP).  You will need to bring a copy of that referral the day of your scheduled surgery.  This will be filed along with our claim form on your behalf. 

We will submit all claims pertaining to your surgical procedure performed at this facility only.  All other services provided, i.e. physician, pathology, and/or anesthesia will be billed by their own administration or billing departments.
